I would say that harvesting organs is different as ppl in need of an organ transplant may very well receive organs due to natural death. Also illness is different to a fatal accident. If someone falls ill then they only have a right to what help is available by non criminal means. In the trolley question I would switch tracks and do everything I could to untie the person. The reason I would switch tracks is because there is a better chance at freeing one person then five.
