PAWTUCKET, R.I. (AP) -- A nursing home in Pawtucket is being investigated after an elderly woman restrained in a wheelchair was found dead. The investigation into the Oak Hill Nursing and Rehabilitation Center is being conducted by the state Health Department. The woman died April 21st.
