Rockford/Freeport, Illinois- The Illinois Department of Public Health has fined two area nursing homes for supervision violations. The Courtyard Terrace Nursing Home, 2313 N. Rockton Ave., in Rockford was fined $10,000 for inadequate supervision after discovering some residents had unknown injuries.
