Widow given fatal painkiller dose after nurse mixed up two


An elderly widow died after a nurse mixed her up with another patient and gave her a fatal dose of the wrong medication. Sylvia Roberts was the victim of a 'cavalier' approach by hospital staff, a coroner ruled.
