Turning & Repositioning of LTC Residents
How can you be sure that 144 residents are being turned and repositioned every 2 hours?

Repositioning
Immobile residents should be turned and repositioned every 1 and half to two hours. if they are in bed you can note how they are positioned right after they have been put to bed, then check them 2 hours later.
If they are in wheelchairs: one LTC facility I worked the nurses would audit by placing tags under some of the residents. When they are supposed to be toileted or repositioned, the CNA will see the tag and give it immediately to the Charge Nurse. The tag system actually works for residents in bed too. Of course this audit system would have to be approved by the DON first.
