notifying transplant coordinators of patients admission
I work in a renal ward and we are having problems with patients who are on the transplant list being admitted to hospital without the transplant coordinator being aware of this. Thus they are not being suspended from the list if need be and also the transplant coordinator is unable to trace them should a donor become available thus wasting valuable time that could be used for someone else.
Does anyone have suggestions? What do you do in your clinical area to ensure this isn't happenein? All suggestions welcome
